Request for Accommodation Form

Student Accommodation Request Form

All students applying for disability services must complete the "Request for Accommodation Form". This form needs to be submitted to the Department of Diversity, Equity and Inclusion attention: Stephanie Price, Director of Equity, EEO and University Accessibility Services. Processing for your request may take up to 4 weeks.

MUSC Medical Verification Clinical Form

MUSC’s Medical Verification Clinical Form should be completed by the student’s qualified healthcare provider when requesting accommodations in the clinical setting. The student’s healthcare provider will need to complete this form as well as provide supporting documentation which should include testing and/or evaluations to support the student’s diagnosis. Students looking to apply for accommodations will also be required to complete the Student Accommodation Request Form. Accommodations approved for the clinical setting do not automatically transfer to the classroom (didactic) setting, therefore a new request for accommodations should be submitted to the Director of Equity, EEO, and University Accessibility. Processing time may take up to 4 weeks.

Immunization Exemption Request for Accommodation Form

Students seeking a medical and/or a religious exemption from getting the required vaccinations will need to complete and submit one or both of the Immunization Exemption Request for Accommodation Forms listed below. Please complete the form in its entirety. Incomplete submissions may result in your request being denied. Your completed form will be handled by a representative in the department of Diversity, Equity, and Inclusion. Processing time may be up to 4 weeks (depending on the volume of requests). Furthermore, if your program contains a clinical component, your vaccination status may limit your access to some clinics and therefore could impact your matriculation through your program.
