How To Request Accommodations

The Office of Federal and State Compliance Office facilitates services and accommodations for students and applicants with disabilities.

Examples of covered conditions include but are not limited to: Attention Deficit/Hyperactivity Disorder (ADHD); autism spectrum disorders; chronic medical conditions (cystic fibrosis, diabetes); learning disabilities; mental health disabilities (bipolar disorder, depression, post-traumatic stress disorder); mobility disabilities (use of wheelchair or other assistive device); neurological disorders (traumatic brain injury, epilepsy, motor or tic disorders); orthopedic disabilities (bone or muscle disorders, connective tissue disorders); sensory disabilities (hearing, vision).

Please complete the following steps to request accommodations:

  1. Complete and submit the Self-Disclosure and Request Form for the academic setting in which you are requesting accommodations. The self-disclosure form gives you the opportunity to provide additional information about your disability, the impact of the condition, the requested accommodations, and any accommodations you have received in the past. If you do not have a NetID yet, please email ada-support@musc.edu for a PDF version of the Self Disclosure and Request Form.
  2. Submit current and comprehensive disability documentation that meets the standards established by the Office of Federal and State Compliance’s Documentation Guidelines. Students will be required to re-submit supporting documentation for each academic setting. If you are requesting accommodations for a didactic setting, please have a healthcare provider complete the Didactic Form. If you are requesting accommodations in the clinical setting, please have a healthcare provider complete the Clinical Form. Your healthcare provider must be qualified to make the reported diagnosis, and the submitted documentation must be current. Documentation can be:
    • Securely uploaded online with or after submitting the Self-Disclosure and Request Form;
    • Faxed to 843-792-1288; or
    • Mailed or delivered to the Office of Federal and State Compliance at 171 Ashley Ave, Main Hospital, Suite 246, Charleston, SC 29425.
  3. Meet with the Office of Federal and State Compliance for an initial assessment interview. Please note, we may separately consult with subject matter experts in their respective health fields regarding accommodation requests.

Although you may request disability services at any time, we strongly encourage you to submit your request with your disability documentation as early as possible. Doing so will facilitate the timely provision of accommodations. Please note, processing your request may take up to 4 weeks.
